• Student Registration Information
     

    Student registration is processed at the Centennial School District Administration Building located at 433 Centennial Road, Warminster, PA 18974.

    Student registrations may be taken online or in-person. 


    Online Registration  

    1.  The parent/guardian must have an email address to use the online registration feature in Skyward.  
     
    2.  To use the online registration feature in Skyward, the parent/guardian must have a Skyward login.

    3.  A parent/guardian registering a student online will need to present the following documents to the Registrar to complete the registration process:
    • State-issued Birth Certificate or Baptismal Certificate
    • Immunization Records
    • 4 Proofs of Residency 

     4.  Once the student is registered online, make an appointment with the Registrar to present the student's Birth Certicate, Immunization records, and 4 Proofs of Residency to finalize registration.

     
    In-person Registration 
    1.  Follow this link to make an appointment. All required documents must be presented to the Registrar to finalize the registration process.
    2.  The registration forms should be downloaded and completed prior to the appointment or you may use a computer kiosk at the Administration Building to complete the online registration process. If using our computer kiosk, please arrive 10-15 minutes prior to the scheduled appointment time.

    Student Registration Process

    Information Required for Registration
    Students must be residents of the Centennial School District to register for school. The following documents are required to register a student:
    • State-issued Birth Certificate or Baptismal Certificate
    • Immunization Records
    • 4 Proofs of Residency (one must be a lease, mortgage statement, deed, or agreement of sale)
    • Student Disciplinary History (for students in Grades 6-12)
    If the student is temporarily living with a guardian or family that resides in the district, contact the Registrar for more information about the registration process.
     
    In addition to the required documents, the parent/guardian registering the student will need to complete the following forms online or in hard copy:
     
     Student Residency Checklist
     Student Registration Information (Only needed if registering by Paper)
     School Health Services History (Only needed if registering by Paper)
     Childcare Arrangements Request Form (Only needed if registering by Paper)
     Release of Student Records Form (Only needed if registering by Paper)
     Student Discipline History (Only 6th-12th Grades) (Only needed if registering by Paper)
     Home Language Survey
     Pennsylvania Department of Health Physical Form
     Pennsylvania Department of Health Dental Form
     A copy of the student’s Individual Education Plan (IEP), 504, Gifted Education Plan (GIEP), Custody Agreement (if applicable).
     
     
     

    Student Eligibility

    To register a student for school in the Centennial School District, the student must be a resident of the district. The parent/guardian registering the student must provide 4 proofs of residency in order to register the student. Students transferring from another school will need to provide transcripts and complete a Student Discipline History form (for Grades 6-12 only). The parent/guardian can complete a release of records form so that the Registrar may obtain these documents from the former school. If the student has an individualized education plan (IEP), gifted individualized education plan (GIEP), or 504 Service Plan, any of these documents must also be presented at the time of registration.

    Kindergarten/Grade 1

    The Centennial School District policy establishes the entry ages for Kindergarten and Grade 1. Student must be 5 years of age by August 31st to register for Kindergarten. Students must be 6 years of age by August 31st to register for Grade 1.


    Withdrawing from School
    Parents/guardians wishing to withdraw a student from school must meet with the Registrar at the Administration Building. To withdraw a student from school, make an appointment with the Registrar or call 215-441-6000 Ext. 11035.
