Joint/Shared Custody Transportation Arrangement

Parents who share legal custody of a student are to complete the Joint/Shared Custody Transportation Arrangements Request form. This form is used to request transportation to both parents' homes. Parents please follow the guidelines listed below for making a request. Requests should be received by August 1st and submitted every school year.

  1. Legal shared custody, with student residing in two (2) different District households and copy of agreement on file in the school office.

  2. Both addresses must be within the Centennial School District. If either parent wishes to have their child dropped off or picked up at an alternate care site, a Child Care Form must be completed and initialed by both parents agreeing to this assignment. The alternate care site must be within the boundaries of their school of attendance.

The Principal will review requests by parent(s)/guardian(s) residing in the District with shared legal custody of a student and will decide on a case-by-case basis in consultation with school administration and the school solicitor. The Transportation Supervisor will determine if the request is approved or denied. The form will then be completed and returned to the building principal. The school will inform the requesting parent/guardian of the decision and busing information.

In emergency situations, the parent/guardian will complete and submit the Joint/Shared Custody Transportation Change Request form to the Principal, who will approve or deny the request and inform the Transportation Supervisor. If time does not allow a written request prior to the change, the request form should be completed and submitted to the Principal as soon as possible following the change.
