• New Hire/Pre-Employment Required Paperwork & Clearances Information
     
    Congratulations and welcome to the Centennial School District!  Below you will find important information and forms for you to review and complete before you can begin employment.  Candidates are strongly encouraged to complete these forms as soon as possible after receiving an offer of employment.  
     
    Please follow the CHECKLIST (below) and complete the required paperwork for employment with Centennial School District.

    ALL NEW EMPLOYEES MUST COMPLETE THE FOLLOWING PAPERWORK PRIOR TO EMPLOYMENT:
     

    On October 22, 2014, Governor Tom Corbett signed into law Act 153 of 2014, which significantly changed the background check requirements for new and current school employees, contractors, and volunteers. All new employees must obtain clearances prior to employment.  The law, amended on July 1, 2015, requires current employees to obtain new clearances every 60 months.  

     
     
    1.  Clearances (Important Note About Clearances: All of the required clearances listed below must be dated within 60 months/5 years of submission. For individuals whose FBI Criminal History Report is older than one year, individuals must submit a paper copy of the unofficial Criminal History Record Information (CHRI) report on Pennsylvania Department of Education (PDE) letterhead. TB test results must be dated within one year of submission.)
      1. Click on “Create Individual Account.”
      2. You must create a “Keystone ID” and follow the step-by-step directions for receiving a temporary password by email, and then creating a new password.  Be sure to keep your Keystone ID as you will need this to log back into your account to create a new password.
      3. When results are available, you will receive an email.  Log back into your account and view the official results.
      4. Save the results page to your computer and also print a copy for your records.
      1. Click “Submit a New Record Check (requires a credit card)”
      2. Follow step-by-step directions on the site.
      3. Results will be available within a few minutes.
      4. When results are ready, click the Control number, which will bring up a registration receipt – this is not the official State Police Clearance.  Please click the “CERTIFICATION FORM” link which appears below the record check details. 
      5. Save this certificate to your computer and print a copy for your records.
      • (1) Register on-line and schedule your fingerprinting appointment.  Submit the registration receipt with the UE ID number, or submit your unofficial report copy to Human Resources; (2) Report to the designated fingerprinting site to have your fingerprints submitted electronically.
     
     
     
    • Physical Exam (Must be dated within one year of submission.)
